Can a modem/router furnished to you by your ISP be used with another
ISP, or would such a device be locked by the original ISP and
therefore limited to them only?

The answer is sometimes you can and sometimes you can't.
Some vendors install custom firmware locking the device to their
network, much like most USA phone carriers lock cell phones to a network.

The above box you have is a DSL modem with combined router.
If your existing Earthlink connection is DSL then you might have a chance.

I would first follow the instructions on how to default the box to the
current firmware's default settings, or as far as the firmware will let you.

With the box defaulted you should be able to log into it using the
username and password of admin, although there are other
username/password combinations that might be in the box if the original
vendor installed custom firmware.

If the box has the actual factory firmware in it you should then be able
to follow the Quick Start Guide and get the box to work.

you included, but nothing worked. It looks like this device was

Your right Kirk_Von_Rockstein, I forgot some models have crappy
auto-search for the VPI/VCI settings. Entering them in manually might
bring up the username / password prompt for the current DSL account.

can't log in because it won't accept the usual username/password
combinations (admin/admin - 1234/1234 - root/root - cisco/(blank) - or
a dozen others. In wifi (that is, without connecting the device to
laptop with ethernet cable), it won't accept the password that is
shown on the bottom, even after a reset.

But thanks anyway.

Did you try ?:
admin/<nothing>
admin/1234
admin/1111

Did you try going to Google with this search text ?:
default admin password cisco linksys router

(replace "cisco linksys" string by your modem model ...)

Rambo, I tried those also, but no luck. Did the google search but
none of the suggested lists worked either. Did the 30-30-30 sequence,
didn't change anything either.

I've come to the conclusion that the original ISP (Fairpoint) put
firmware on it that has hardcoded username/password that don't change
even with a 30-30-30.

But thanks anyway.

"It depends."

There are technical implications / limitations that /may/ prevent it.

There may also be legal implications / limitations that /may/ prevent
it. - Who owns the modem and / or router? Do you? Or did the ISP
loan it to you as part of your service with them?
